Mitral valve cerclage annuloplasty를 위한 환자맞춤형 심실 모델의 전기전도수축 컴퓨터 시뮬레이션
Mitral valve cerclage annuloplasty (MVCA) is a newly introduced catheter-based treatment to avoid the clinical risk of the open-heart procedures to repair a cardiac valve disorder. We developed a patient-specific cardiac electromechanical model of pre- and post-MVCA to better understand physiological mechanism through which mitral valve cerclage annuloplasty treatment may contribute to cardiac recovery. Furthermore, it is to establish a virtual test procedure before treatment using this simulation technique. In this study, three-dimensional (3D) cardiac models of pre- and post-MVCA at the end diastole for each patient were established by applying electromechanical governing equations to the model geometries. We also tested the virtual post-MVCA model by more tightening the diameter of ventricular basement than the actual post-MVCA model without heart recovery. We computed ventricular pressure, ventricular volume, the ejection fraction, stress distributions loading on the ventricular muscle by using the patient-specific electro-mechanics ventricular simulations. Ventricular stress distribution changes from pre-MVCA to post-MVCA follow-up during seven months. Our results showed correlations between ventricular stress distributions and heart recovery by comparing the simulation results and clinical outcome. In conclusions, this simulation has a potential for pre-MVCA virtual test tools by using correlation between MVCA-derived electromechanical changes and the subsequent heart recovery.
