드롭랜딩 시 전방십자인대의 대퇴골 접합부 손상 위험도 고찰

It has been reported that anterior cruciate ligament (ACL) femoral attachment site is more susceptible to injury than its tibial attachment site. This appears to be associated with its acute ligament angle at the femoral attachment site. However, it is not clear why the morphological characteristics at the femoral attachment site increases the vulnerability to failure. In this preliminary study, we developed a 2D fiber-reinforced ACL finite element model to investigate the stress distribution at the femoral attachment site during a simulated drop landing. In the 2D fiber-reinforced model, the ligament was assumed to be isotropic. The model was driven by the projected displacement boundary conditions, which were obtained from an in vitro knee test and the corresponding in silico knee model simulation. The results show that the peak stress concentration occurs at the inferior margin of the ACL femoral attachment site. This study addresses that microdamages in the collagenous ligament fibers could be accumulated at the femoral attachment site and thus result in ACL injuries.
